Genicular Artery Embolization (GAE)

Are you struggling with chronic knee pain from osteoarthritis? You may worry that knee replacement surgery is your only option, but there are newer treatments that can provide relief with less discomfort, scarring and downtime. At Minimally Invasive Medical Group, we use genicular artery embolization (GAE) to reduce knee pain and help you avoid joint replacement surgery.

What Is Genicular Artery Embolization?

Osteoarthritis is a frequent cause of knee pain, especially in later life. After decades of motion, natural wear and tear reduces the protective cartilage that covers your bones and cushions the joint.

Without protection, the bone ends catch and rub, creating inflammation. The inflammation causes long-term joint pain that steadily gets worse. The knee joint becomes stiffer and weaker as the condition progresses, and you may eventually lose mobility and function.

Genicular artery embolization blocks some of the blood vessels that contribute to these problems. Reducing blood flow into the joint helps decrease inflammation and pain, giving you relief from discomfort without the need to remove or replace any part of your knee.

GAE is safe and effective, lasts for years and is covered by medical insurance. You can even have the procedure after knee replacement surgery if you still experience pain.

How Does GAE Work?

During a GAE procedure, the interventional radiologist makes a small puncture in the skin to access the artery. Using X-ray imaging, they guide a thin, flexible tube (catheter) toward the genicular arteries that feed inflammation in the knee joint and release tiny particles called microspheres.

The microspheres block the passage of blood through the vessel. This causes the artery to shut down so less blood flows to the inflamed areas of the knee. Healthy circulation to the rest of the knee is not disrupted.

Knee osteoarthritis is a progressive condition that becomes increasingly painful. Knee replacement is often the suggested solution, but you may not be a good candidate for surgery, or you may simply not be interested in an invasive procedure.

GAE is an alternative option for reducing knee pain with many additional benefits:

  • Minimally invasive, outpatient procedure
  • No large incisions
  • No removal of bone
  • No general anesthesia
  • No overnight hospital stay
  • Minimal downtime
  • Targeted treatment that only affects the problematic area
  • Lower risk of complications compared to invasive surgery
  • No age, weight or health condition restrictions
  • Preserves your natural knee joint
  • No effect on candidacy for future knee replacement
  • Long-lasting pain relief that can extend for months or years
  • Covered by Medicare

How Does GAE Work?

Before recommending a treatment plan, our expert team carefully evaluates each patient’s symptoms, medical history and goals. While GAE can effectively reduce knee pain for many people with osteoarthritis, we always start with a consultation to ensure it is the best course of action for you.

GAE is a versatile treatment. Unlike knee replacement surgery, GAE has no strict age or weight limitations. It can be an excellent option for patients who have already had knee surgery but still experience pain, as well as those who want to preserve the option for knee replacement in the future.

Get Your Treatment Plan

You may be a good candidate for genicular artery embolization if you:

  • Experience chronic knee pain from osteoarthritis that affects your daily activities
  • Have not found adequate relief from conservative treatments like medications or physical therapy
  • Have tried steroid injections but found the relief
too temporary
  • Do not wish to use medications long-term
  • Want to avoid or delay knee replacement surgery
  • Are not a good candidate for knee replacement
  • Desire a solution with minimal downtime

What to Expect During Treatment

The OCIR team provides a comfortable, well-organized experience from start to finish. The full visit typically takes a few hours, with the GAE procedure itself lasting about 60-90 minutes.

On the day of your procedure, you will arrive at our modern outpatient facility where our team will help you get settled. We will place an IV line to administer moderate sedation and any necessary medications to keep you comfortable. We will also clean the treatment area in your upper thigh and numb it with local anesthesia.

During the procedure, you will lie comfortably on your back while our interventional radiologist uses the catheter and advanced imaging technology to target the vessels causing inflammation in your knee. While you will be awake during the procedure, the sedation helps you feel relaxed, and the anesthesia ensures you feel nothing more than slight pressure. You will be always be able to communicate with your doctor and medical team.

After the procedure is complete, you will spend some time in our recovery area while the sedation wears off. Our team will come in periodically to check your vitals and the procedure site until you are deemed safe to discharge.

Genicular Artery Embolization Recovery

Most patients go home within a few hours of the procedure. You will have a small bandage at the access site in your upper thigh, but no stitches are needed. You might experience some mild soreness at this site for a day or two.

Our team will provide detailed instructions for caring for the access site and any other recovery guidelines you should follow. Many patients return to desk work within one to two days and light activities within one week. We will work with you on an appropriate timeline for returning to more strenuous activities based on your individual healing response.

You may notice some improvement in your knee pain right away. However, the full benefits of GAE develop over time, as the inflammation in your knee eases. You should feel the pain starting to lessen after two weeks and significantly after one month.

FAQ'S

Got Questions? We Have Answers

What are the symptoms of knee arthritis?

Knee arthritis develops gradually, with symptoms that become more noticeable over time. Common signs include:

  • Pain that worsens with activity but improves with rest
  • Morning stiffness that takes 30 minutes or more to improve
  • A grinding or clicking sensation when moving your knee
  • Swelling around the joint, especially after activity
  • Difficulty with everyday movements like climbing stairs or getting up from a chair
  • Knee weakness or instability, particularly on uneven ground
  • Pain that tends to be worse in rainy or cold weather

You might notice that your symptoms are mild at first but become more pronounced as the day goes on, especially after you’ve been active. You might also find that your knees feel more stiff and painful after sitting for long periods, such as during a car ride or working at your desk.

Knee OA symptoms can vary from person to person, and you may not experience all of them. If you’re noticing any of these signs, particularly if they’re affecting your daily activities, it’s a good idea to discuss your treatment options with our team.

Is genicular artery embolization right for me?

When knee osteoarthritis first develops, you might manage it successfully with pain medication and physical therapy. Anti-inflammatory steroid injections and/or lubricating hyaluronic acid shots can help as the pain gets worse. But nothing stops the disease from progressing.

If you can’t manage knee pain with these treatments, another option is joint replacement surgery. This involves removing the damaged bones in your knee and replacing them with artificial parts.

However, some people may not be good candidates for knee replacement, and others may be reluctant to consider a major operation involving prolonged pain and healing. Genicular artery embolization is an effective, safe alternative to knee replacement surgery.

How long does genicular artery embolization last?

Although GAE is not a cure for osteoarthritis, it can provide long-lasting relief. Large studies show that two-thirds of patients have significant improvement in knee pain after GAE, which can last for several years.

What is the success rate of genicular artery embolization?

Studies have found GAE to be very effective. Patients whose knee pain is more severe may respond better to the procedure. Additionally, studies have found that most patients experience no side effects or only minor side effects from the treatment.

Does insurance cover genicular artery embolization?

Most health insurance plans cover GAE. We recommend contacting your provider to confirm the extent of your coverage and the anticipated out-of-pocket costs before your procedure. Our staff will work with you to ensure you are able to get the care you need.

What doctor performs genicular artery embolization?

GAE is performed by an interventional radiologist, a physician who specializes in using minimally invasive treatments involving image guidance to diagnose and treat diseases.
