What Is Y90 Radioembolization?

When dealing with liver cancer, finding an effective treatment that minimizes side effects is crucial. At the Orange County Interventional Radiology Center in Lake Forest, Mission Viejo and Irvine, we offer Y90 radioembolization — a cutting-edge, minimally invasive treatment for both primary and secondary liver tumors. This targeted therapy delivers radiation directly to cancerous tissues while sparing healthy liver cells, providing hope for patients who may not be candidates for surgery or who haven’t responded well to conventional treatments.

Y90 radioembolization is a minimally invasive procedure that combines embolization and radiation therapy to treat liver tumors. It uses tiny glass or resin microspheres containing the radioactive isotope yttrium-90, which are delivered directly to the tumor’s blood supply. This two-pronged approach works by both blocking the blood vessels that “feed” liver tumors and delivering a therapeutic dose of radiation to destroy cancer cells from within.

Ideal Candidates for Y90 Radioembolization

For patients with liver cancer, treatment options can sometimes feel limited, especially when surgery isn’t possible. Y90 radioembolization offers an effective alternative for those with multiple liver tumors or for whom conventional treatments haven’t been successful. You may be an ideal candidate for Y90 radioembolization if:

  • You have primary liver cancer (hepatocellular carcinoma) or secondary liver tumors
  • Your tumors cannot be surgically removed
  • You haven’t responded well to chemotherapy or other treatments
  • You need tumor control while waiting for a liver transplant

Before recommending Y90 radioembolization, our interventional radiologists will conduct a thorough evaluation of your medical history, liver function tests and imaging results. This comprehensive assessment helps ensure the treatment is appropriate for your specific condition and will provide maximum benefit with minimal risk. We also work closely with your oncology team to integrate this therapy into your overall cancer treatment plan.

What to Expect During Treatment

Y90 radioembolization is typically performed in two stages. The first visit involves a mapping angiogram to visualize the blood vessels of your liver and identify the best approach to your tumors. Our team will also block any vessels that might allow radiation to reach other organs.

During treatment, you’ll receive mild sedation and local anesthesia to keep you comfortable. Our interventional radiologists will make a small incision in your groin to access the femoral artery and use real-time imaging to guide a catheter into the hepatic artery — the main blood vessel feeding liver tumors. Once in position, thousands of Y90 microspheres are released, lodging in the small vessels surrounding the tumors and emitting radiation to destroy cancer cells.

Y90 Radioembolization Recovery

Compared to more invasive surgery, recovery from Y90 radioembolization is relatively quick. However, you may experience fatigue, mild abdominal pain, loss of appetite or nausea for several days after treatment. Depending on the dose received, you may also need to limit contact with other people to avoid exposing them to radiation. Most patients can resume normal activities within a week. Our team will schedule follow-up imaging at regular intervals to monitor the treatment’s effectiveness and determine if additional sessions are needed.
